We use your information to schedule and deliver care, communicate about appointments and services, respond to your inquiries, and improve our programs. Protected health information is handled in accordance with HIPAA.
SMS / text messaging
If you provide your mobile number and opt in — by checking an SMS consent box on a form, texting us, or expressly requesting text communication — we may send you messages related to your appointments (reminders, confirmations, changes), care coordination, account notifications, and service updates from Precision Transformation.

- Consent to receive SMS is not a condition of receiving care or purchasing any service.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors that support the SMS program (for example, our messaging platform provider) is permitted only to deliver the service you requested. All other categories of use exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Where it's stored
Information is stored in access-controlled systems used by Precision Transformation and available only to authorized team members and vetted subcontractors bound by confidentiality obligations.
Your choices
You can request a copy of the information we hold about you, or ask us to delete it, by contacting us. To stop SMS messages, reply STOP to any text.
